Provider Members Dues (Medicaid/Medicare, commercial Insurance, etc.)
All agencies will fall within the following levels or tiers for dues payment:
- Minimum Dues - Revenues up to $217,865 – Minimum Dues of $572.00
- Tier 1: Revenues from $217,866 to $2,000,000
- Tier 2: Revenues of more than $2,000,000 but less than $7,500,000
- Tier 3: Revenues of more than $7,500,000
In each tier a cap or maximum dues has been established.
Please use the following formula based upon the level or tier within which your agency’s gross client revenue falls:
Tier #1 - Revenues $217,866 to $2,000,000 (If revenues are more than $2 million, go to tier #2)
Gross client revenue ($_______________) multiplied by .002626 = $____________ (*this amount should not exceed a cap of $5,252 for this category - Example: an agency with client revenues of $1,000,000 will pay $1,000,000 times .002626 for total of $2,626 in dues.)
Tier #2 - Revenues of $2,000,000 up to $7,500,000 (If revenues are more than $7,500,000, go to tier #3)
Pay Base Rate of $5,252 Plus the following: Gross client revenue in excess of $2,000,000 ($_______________) multiplied by .000328 = $____________ (*this amount should not exceed a cap of $7,036 for this category - Example: an agency with client revenues of $5,000,000 will pay the base rate of $5,252 plus the following based on $3,000,000 in revenue in excess of the first $2 million ($3,000,000 times .000328) = $984 for total of $6,236 in dues.)
Tier #3 - Revenues of more than $7,500,000
Pay Base Rate of $7,036 Plus the following: Gross client revenue in excess of $7,500,000 ($_______________) multiplied by .000201 =$____________ (*No matter what this amount is, the maximum dues payment is $7,772. Example: an agency with client revenues of $10,000,000 will pay the base rate of $7,036 plus the following based on $2,500,000 in revenue in excess of the first $7.5 million ($2,500,000 times .000201) = $502 for a total of $7,538 in dues.)
